A Home Care Package (HCP) is available to Australians aged 65 years and over and is designed to help older people get the support they need to stay in their own homes. The government provides funding to pay for services which are personalised based on what each person needs. The program aims to help seniors live comfortably and independently for as long as possible.
To get a Home Care Package a person need to have an Aged Care Assessment Team (ACAT) Assessment. An ACAT Assessment is an individual assessment to determine the level of support and services each client may need to remain at home.
Home Care Packages are available in four levels and are designed to assist people with varying needs. This could be as simple as weekly shopping and cleaning services to more complex daily support such as nursing, physiotherapy, and carer support to enable you to remain at home.
When you receive a Home Care Package, you choose the Home Care provider, like Juniper, that meets your individual needs.
Juniper will partner with you to put together a Personalised Care Plan under your Home Care Package, drawing from a wide range of home support services that have the flexibility to maintain your independence and well-being, as you continue to live your life.
