Juniper’s dedicated Home Care Allied Health team has won an in-house award for implementing in-reach services for Transition Care Program (TCP) customers in Bentley, Martin and Karrinyup.
The in-reach efforts relate to the provision of allied health services delivered by physiotherapists, occupational therapists, podiatrists and therapy assistants to Juniper’s TCP customers following a hospital stay, injury or illness.
Now in place at Juniper Charles Jenkins in Bentley, Juniper Hayloft in Martin and Juniper TCP Karrinyup, the innovative approach can be increased as needed to meet the needs of customers and supports efforts to consistently meet measurable targets implemented by the Department of Health.
To recognise the Home Care Allied Health team’s outstanding efforts, the group received a Game Changer Award as part of the Juniperks Reward and Recognition program, providing a well-earned social get-together.
Further benefits of the in-reach approach include a well-structured support network for all allied health staff working in TCP as well. It has also fostered excellent teamwork and friendships between colleagues from TCP residential allied health and community allied health teams.
With expert guidance from team leaders, the Home Care Allied Health team took up the challenge of introducing its services to a different environment and have ultimately excelled themselves in improving our customer’s outcomes.
