Juniper expands TCP to Karrinyup with innovative small house model

Juniper is proud to announce the expansion of our Transition Care Program (TCP) to Karrinyup, introducing an innovative small house model. This model provides a home-like environment where customers can benefit from 24/7 care during their recovery.

TCP Karrinyup consists of two homes, each featuring seven bedrooms with ensuites, and shared kitchen and living spaces. These communal areas allow customers to engage in everyday tasks, promoting their recovery journey in a comfortable and familiar setting. The first home opened Tuesday, July 23, 2024, with the second following shortly thereafter.

TCP provides short-term care for up to 12 weeks post-hospitalisation, either at a residential home, your own home, or both, with a focus on improving health and mobility for independent living. For those needing longer-term support, the program helps explore different living arrangements and collaborates with multi-disciplinary teams to achieve individual wellbeing goals.

Under the TCP Karrinyup small house model, customers will benefit from round-the-clock care by a Registered Nurse, as well as support from a team of multi-skilled carers and visits from Juniper’s team of allied health professionals, including physiotherapists and occupational therapists.

Juniper CEO Russell Bricknell said the innovative small house model would go a long way to helping people regain their daily independence and enhance their quality of life.

“Unlike some of the traditional Transition Care offerings, a small-house model can benefit many of our customers who aren’t quite ready to go home after their hospital stay but would benefit from being able to practice everyday tasks in a home-like environment whilst receiving expert clinical and personal care,’ Russell said.

The TCP small house model also encourages social interaction and a sense of community, crucial for maintaining mental and emotional health. Located next to Juniper Chrystal Halliday Residential Aged Care Home, TCP Karrinyup allows customers to participate in activities and social events if they choose.

The expansion of Juniper’s TCP to Karrinyup is part of a broader effort to ease pressure on public hospitals. Care and support at TCP Karrinyup will adopt a reablement approach, a person-centered care strategy focused on helping individuals regain independence and improve functional abilities following an illness, injury, or hospital stay. This approach emphasises developing skills and confidence needed for daily activities, ultimately enhancing the quality of life and promoting long-term well-being.
