New Juniper Annesley TCP opens its doors

The first customer has arrived at the newly created Juniper Annesley Transition Care Program (TCP) in Bentley. The 16-bed location provides a home-like environment where customers can benefit from 24/7 care during recovery following a hospital stay, injury or illness.

Opening of the fresh and welcoming Annesley TCP setting is the latest addition to Juniper TCP services already in place at Juniper Hayloft in Martin, a small-house model at Juniper TCP Karrinyup and support in people’s own homes in the community.

Juniper TCP provides short-term care for up to 12 weeks post-hospitalisation, either at a residential aged care home, a person’s own home, or a combination of both, with a focus on improving health and mobility for independent living. For those needing longer-term support, the program helps explore different living arrangements and collaborates with multi-disciplinary teams to achieve individual wellbeing goals.

With a modern feel and look, Juniper Annesley TCP has a shared kitchen and living spaces. These communal areas promote social interaction and enable customers to resume their everyday tasks in a comfortable and familiar setting.

Customers will benefit from round-the-clock care by a Registered Nurse, as well as support from a team of multi-skilled carers and visits from Juniper’s team of allied health professionals, including physiotherapists and occupational therapists.

Juniper Annesley welcomed its first customer on day one of operations on Tuesday 1 October. The fit-for-purpose location is expected to reach capacity within two weeks.

Juniper Executive Director Community Business Development Jo Wilkinson said it was exciting to welcome customers to the homelike Annesley setting, which was focused on helping customers get back to what they normally do.

“The setting provides the option for customers to get up and about to make breakfast or a coffee, like they would at home,” Jo said. “We have no rails on the walls, as it’s about prescribing equipment that is individual to each customer.”

Juniper Annesley TCP is part of a broader effort to ease pressure on public hospitals with a reablement approach and person-centred care strategy to help customers regain their independence and improve functional abilities.
